Social Determinants of Health (SDoH) Optimization
CMS now requires mandatory reporting of SDoH for food insecurity, interpersonal safety, housing insecurity, transportation insecurity, and utilities as is it recognized these factors affect a wide range of health functioning and quality-of-life outcomes and risks. Optimization of the screening form ensures we are using a validated tool that supports SDoH reporting to CMS.
When is the go-live?
Use of PRAPARE for SDoH Screening will be implemented on February 3, 2025. |
Who is affected?
The change will affect the following venue(s):
- Acute/Inpatient (to include ED & Peri-Op)
- Acute Care Management
- Ambulatory
At the following NLH Member Organization(s):
- All NLH Member Organizations, excluding NL Mayo Hospital

Why implement?
To meet documentation requirements, this screening must be completed annually in the ambulatory setting and with every inpatient/ED encounter. To improve workflow efficiency, all required information is now located on one form, eliminating the need to go to multiple places to complete the screening.
An MPage component will be added to increase visibility of current and previous documentation and a new auto text allows providers to easily pull the screening into their notes. The final enhancement automates the firing of appropriate Z codes to the problem list for any positive screening. |
